We require a Care Co-ordinator for our domiciliary care office in Colchester, Essex.

Must have NVQ Level 3 or be willing to work towards it

The successful applicant will need to have experience working in the home care sector, preferably previous experience working with vulnerable adults.

STANDARDS OF PERFORMANCE:

KEY RESPONSIBILITIES

· Produce weekly rotas for all staff in accordance with the service user needs and choices

· Ensure Care Needs Assessments and information folders are in place at the start of all new services

· Ensure Support Plans and Risk Assessments are in place within a timely manner of the start of the service and in line with organisational policy

· Ensure service user reviews are carried out at the appropriate time in line with organisational policy

· Ensure all service users’ needs are being met and their choices are being respected by communicating with service users and staff

· Ensure services have been co-ordinated appropriately to respect the choice of times, staff and tasks

· Communicate any concerns to relevant individuals and ensure concerns have been actioned and resolved

· Rotated On-Call duties

OTHER RESPONSIBILITIES AS REQUIRED

· Stand in when the Registered Manager is absent, this may include also taking responsibility for the manager's phone when the manager is away (this does not apply every time the manager is off)

· Cover duties in severe emergencies or when a support worker goes off sick until regular workers can pick up. (this action can only be sanctioned as a final measure)

· Assist the manager as required during times of extreme pressure (the manager must take into account that regular duties will not be carried out during these times and regular duties must always take priority)

COMPLIANCE

1. Observe legislative requirements according to the Working Time Regulations

2. Observe Service User and Support Worker Confidentiality

3. Observe Delight Supported Living Ltd Policy in relation to issues surrounding confidentiality

4. Ensure GDPR are followed

ADMINISTRATION

1. Maintain accurate records for Support Workers and Service Users. Record relevant details of all new Support Workers and Service Users according to Delight Supported Living Ltd Policy and kept all records updated using a computer and manual systems

2. Become competent in all areas of office management, with exceptions only at the Registered Managers discretion
