Based at The Point and working across Doncaster communities 

Role? 

Support community assets (darts, Cast and Heritage Doncaster) and researchers to identify and engage with individuals with lived experience of ill health and support them to play an active part in the programme, either by accessing activity and/or engaging in the research. 

Identify the barriers and enablers to participation and engagement in art, culture and creative activities for those with the highest risk of poor health.? 

Improve the profile of Arts & Health programmes so that they are inclusive, widely used and valued by both residents and health professionals.?? 

Objectives? 

Successfully enable individuals and communities experiencing poor health to engage in a wider and more accessible range of art, culture and creative activities provided by community assets.? 

Improve the profile of Arts & Health programmes so that they are inclusive, widely used and valued by both residents and health professionals in Doncaster.? 

Responsibilities? 

Work with community assets (darts, Cast and Heritage Doncaster) and researchers to identify and engage with individuals with lived experience of ill health and support them to access art, culture, and creative activities. 

Enable individuals with lived experience of ill health to play an active role in the programmes action research projects and Patient Participation and Involvement group.? 

Raise awareness of creative health activities and benefits with key stakeholders, including GP Link workers, Primary Care Network Care Co-ordinators and other health and social care professionals. 

Be an active member of the Arts & Health Board. 

To contribute to an evaluation of the Creative Health Connector post through a ‘test and learn’ process, identifying what is working well and what could work better 

Requirements? 

Demonstrate good knowledge, experience and understanding of Doncaster and/or South Yorkshire. 

Willingness to work flexible hours when required to meet work demands, including occasional weekends and evenings. 

Ability to travel locally on a regular basis?across Doncaster.
