Care Co-ordinator - Farnborough & Camberley
Purpose of Position
An essential role in providing a critical and fluid structure to our Carers on a daily and weekly basis. With a focus on relationships and service delivery, this role will enhance the quality of care provided to our clients by supporting all our Carers to achieve their visits in a timely and consistent way. With attention to varying demands including local areas, business costs, and skill set, this role will require extensive experience which will equate to a Supervisory level.
Principle Responsibilities
- Be responsible for the scheduling of our Home Care Teams across the area and manage communication daily, ensuring all calls are covered and attended.
- Support team members across the area and ensure all schedules are covered by matching Carers with clients’ needs while meeting their preferences regarding days, times, and duration of visits.
- Lead by example, ensuring that any reasonable requests are acted upon and any concerns or complaints are promptly investigated and dealt with appropriately.
- Plan care rounds while focusing on Carers' locations and travelling times when completing the schedule, being aware of how a more efficient care round can benefit both our Home Care Team and the business.
- Apply a positive, solution-focused attitude at all times to enable team targets to be met and exceeded.
- Show commitment to your team by supporting new initiatives and making recommendations for continuous improvement.
- Deal with daily enquiries from new and existing clients, and signpost or provide information about other professional services that may be required.
- Demonstrate a collaborative mindset towards your team and Registered Manager by demonstrating accountability at all times.
- Communicate effectively with the Registered Manager, Deputy Care Manager, Care Supervisors, and other professionals regarding clients and Carers, focusing on actioning responsibly and managing all matters confidentially.
- Ensure all Carers know how to use our care planning software in their day-to-day roles, encouraging and helping towards full compliance.
- Manage alerts on the system to ensure they are actioned.
- Record staff sickness, absence, and holidays.
- Take responsibility for the emergency on-call phone on a rota basis as instructed by the Registered Manager, understanding that this may change and is not a constant structure.
- Cover care calls when necessary in cases of urgent need.
- Assist the Registered Manager, Deputy Care Manager, and Supervisors in being a primary communicator between Clients and Carers.
- Support the Registered Manager by learning and adopting appropriate responses and actions when liaising with queries from Clients and Carers, and liaising with the appropriate external professionals, agencies, and families.
- Take accountability for organizing and supporting work-based learning for newly appointed Carers to ensure sufficient support.
- Attend, shadow, and learn how to arrange and undertake regular supervision with Carers.
- Attend any training courses as deemed beneficial to your job role.
- Ensure all communications are logged in the appropriate communication records.

Desired Qualifications
- NVQ / QCF / RQF II or III in Health & Social Care.

- Strong communication & organisational skills.
- Self-reliance and self-motivation with a collaborative, team-oriented approach to all aspects of work.
- Ability to be resourceful, proactive, and accountable when issues arise.
- Experience in coordinating care schedules.
- Excellent IT skills.
- A clean driving licence with access to your own car.
