The role will be based within the Seagull Project, which houses 36 individuals in 7 houses across Brighton and Hove. Residents have a range of complex support needs including substance use, mental health, physical health and learning disabilities. The project provides a safe and stable home for residents to be able to address any unmet needs and consider their life goals, in order to recover from homelessness and work towards moving into more independent living.
Support Coordinators will have a caseload of around 10-15 residents within the Seagull Project.
Responsibilities:
- Through weekly one-to-one keywork sessions, provide tailored support to residents with diverse needs—including substance use, and mental and physical health conditions.
- Assist residents in managing daily living activities, such as budgeting and developing other relevant essential life and employment skills.
- Plan and facilitate monthly house meetings and workshops that promote budgeting, cooking, and creative skills
- Collaboratively develop person-centred Support Plans with SMART goals, empowering residents to progress toward more independent living arrangements.
- Maintain accurate and up-to-date online records.
- Coordinate and accompany residents to key appointments, including but not limited to GP visits, CGL and probation meetings.
- Work in partnership with local services to provide comprehensive and holistic support.
- Deliver support within a trauma-informed framework, aligning with the objectives of the Local Authority and the Rough Sleeper Strategy.
Requirements:
- Driving licence and access to a vehicle is essential.
- Experience of working with adults with complex needs (substance use and mental health) and relevant qualifications desirable (at least one year).
- Available to work Monday - Friday (9am-5pm).
