About the role:
The Social Work Team works alongside patients, families and carers who need palliative social work support. The role requires working with patients in the inpatient unit and community settings, including patient’s homes.
The team works collaboratively with families and carers, community nursing teams, wider hospice services and external organisations to support people in the advanced stages of illness to access the support they need and plan for the future.
The successful candidate will join a friendly and professional team who make a real difference to patients and their families at the most challenging time. This role allows the time to build trusting relationships and the opportunity to support in creative ways.
About the person:
You will manage your own caseload, therefore the ability to work independently is essential.
We are looking for someone who can support patients, families and carers with sensitivity and compassion.
You will have experience of working with people approaching the end of their life in a health or social care service
You must have a clean driving licence and access to a car as we work across Bristol, South Gloucestershire, and North Somerset.

Assess and plan support, joint working with a qualified social worker where appropriate
Provide emotional support

Salary from £27,061 up to £29,696 annum (FTE), dependent on experience
Tuesday-Friday 7.5 hours per day but open for discussion
