Key Responsibilities

  • Conduct strengths-based assessments and develop personalised, outcomes-focused support plans.
  • Promote independence, choice, and control for adults and carers through early intervention and preventative support.
  • Work collaboratively with multi-disciplinary professionals to deliver integrated health and social care services.
  • Identify safeguarding concerns and lead or contribute to adult safeguarding enquiries, ensuring timely and proportionate interventions.
  • Undertake Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DoLS) assessments, applying Making Safeguarding Personal principles.
  • Maintain accurate, high-quality case records and reports in line with professional standards and statutory guidance.
  • Support service users to access appropriate advocacy services, community resources, and universal services to enhance wellbeing.
  • Engage in regular supervision and professional development, reflecting on practice to continuously improve outcomes for service users.

About You

You are a passionate, resilient, and person-centred practitioner who wants to make a tangible impact in adult social care. You will bring excellent communication skills, sound judgement, and the ability to work collaboratively in a multi-agency environment.

You will also have:

  • A recognised Social Work qualification (Degree in Social Work, DipSW, or CQSW).
  • Social Work England registration (or pending for newly qualified social workers).
  • A clear understanding of the Care Act 2014, Mental Capacity Act 2005, and Making Safeguarding Personal principles.
  • Experience (or placement-based understanding for ASYEs) of working with adults, older people, and those with long-term conditions, mental health needs, or disabilities.
  • A commitment to equality, diversity, and inclusion in practice.
  • Strong organisational, assessment, and report-writing skills.
  • IT literacy, including the ability to use case management systems.
  • An Enhanced DBS clearance.
